Merge Upstream's stable 3.0.21 branch into msm-3.0
This consists 814 commits and some merge conflicts.
The merge conflicts are because of some local changes to
msm-3.0 as well as some conflicts between google's tree and
the upstream tree.

During subsystem restart we need to wait for the WCNSS to come up
and open the smd port before sending any data (command) to it
otherwise the data is lost and it will result in hci_cmd_timeout.
If there was a mechanism in place to issue subsystem_restart on
hci_cmd_timeout, it would result in repetitive subsystem_restart
without this fix.

Upper layers are not acquiring the locks properly, hence wakelock release
timeout is increased to 500 ms. This logic and value is derived from
the implementation for UART transport.

When there are packets available on both the event and data channels,
event packets must be read first. UART-based HCI controllers do this
prioritization before sending data over the serial connection, but
with SMD the HCI driver must serialize packets from the two channels.

The device de-registration when called from different process context
without regristation, doesn't complete because of synchronized timer
delete APIs. Hence timer setup state is checked before deletion to
avoid non returnable code. Also graceful cleanup is ensured.

This patch fixes the issue where a wakelock was not being
released when BT was turned OFF, thus preventing the system
to go in suspend state.

The new Ath3k needs to download patch and radio table,
and it keeps same PID/VID even after downloading the patch and radio
table. This patch is to use the bcdDevice (Device Release Number) to
judge whether the chip has been patched or not. The init bcdDevice
value of the chip is 0x0001, this value increases after patch and
radio table downloading.

This patch against current git adds the hardware ID for the Apple
MacBookAir4,1, released in July 2011. The device features a BCM2046
USB chip. The patch was inspired by the previous modifications adding
support for the MacBookAir3,x.

The function used to read from SMD was causing a deadlock
as it can only be used in interrupt context. As we have
now added tasklet support, we are now reading in tasklet
context, and this patch uses the correct read function
to avoid deadlock.

In case of SMD (Shared memory driver) as HCI transport; once the
Tx packets are written over SMD channel, the corresponding memory
objects were not cleaned resulting in memory leaking.
Now these are appropriately cleaned up to avoid reset because of low
memory in long run scenarios.

This reverts commit b1c43f82c5.
It was broken in so many ways, and results in random odd pty issues.
It re-introduced the buggy schedule_work() in flush_to_ldisc() that can
cause endless work-loops (see commit a5660b41af: "tty: fix endless
work loop when the buffer fills up").
It also used an "unsigned int" return value fo the ->receive_buf()
function, but then made multiple functions return a negative error code,
and didn't actually check for the error in the caller.
And it didn't actually work at all. BenH bisected down odd tty behavior
to it:
"It looks like the patch is causing some major malfunctions of the X
server for me, possibly related to PTYs. For example, cat'ing a
large file in a gnome terminal hangs the kernel for -minutes- in a
loop of what looks like flush_to_ldisc/workqueue code, (some ftrace
data in the quoted bits further down).
...
Some more data: It -looks- like what happens is that the
flush_to_ldisc work queue entry constantly re-queues itself (because
the PTY is full ?) and the workqueue thread will basically loop
forver calling it without ever scheduling, thus starving the consumer
process that could have emptied the PTY."
which is pretty much exactly the problem we fixed in a5660b41af.

it makes it simpler to keep track of the amount of
bytes received and simplifies how flush_to_ldisc counts
the remaining bytes. It also fixes a bug of lost bytes
on n_tty when flushing too many bytes via the USB
serial gadget driver.
