Merge Upstream's stable 3.0.21 branch into msm-3.0
This consists 814 commits and some merge conflicts.
The merge conflicts are because of some local changes to
msm-3.0 as well as some conflicts between google's tree and
the upstream tree.

Input: evdev - fix variable initialisation
Commit 509f87c5f564 (evdev - do not block waiting for an event if
fd
is nonblock) created a code path were it was possible to use retval
uninitialized.
This could lead to the xorg evdev input driver getting corrupt data
and refusing to work with log messages like

Input: evdev - Add ioctl to block suspend while event queue is not empty.
Add an ioctl, EVIOCSSUSPENDBLOCK, to enable a wakelock that will block
suspend while the event queue is not empty. This allows userspace code to
process input events while the device appears to be asleep.
The current code holds the wakelock for up 5 seconds for every input
device and client. This can prevent suspend if sensor with a high data
rate is active, even when that sensor is not capable of waking the
device once it is suspended.

Input: evdev - Don't hold wakelock when no data is available to user-space
If there is no SYN_REPORT event in the buffer the buffer data is invisible
to user-space. The wakelock should not be held in this case.

usb: gadget: android: Honor CONFIG_USB_GADGET_VBUS_DRAW
The maximum current draw was hard coded to 500 mA. composite.c
has code that uses CONFIG_USB_GADGET_VBUS_DRAW to set the
bMaxPower and to set whether or not the device is self-powered if
they haven't been set. This change removes the code in android.c
to allow composite.c to set them.

usb: gadget: Fix usb string id allocation
Don't reset next_string_id every time the gadget is enabled, this makes
the next strings allocated overwrite strings allocated at probe time.
Instead, fix rndis not to allocate new string ids on every config bind.

nl80211/cfg80211: Make addition of new sinfo fields safer
Add a comment pointing out the use of enum station_info_flags for
all new struct station_info fields. In addition, memset the sinfo
buffer to zero before use on all paths in the current tree to avoid
leaving uninitialized pointers in the data.

cfg80211/nl80211: Send AssocReq IEs to user space in AP mode
When user space SME/MLME (e.g., hostapd) is not used in AP mode, the
IEs from the (Re)Association Request frame that was processed in
firmware need to be made available for user space (e.g., RSN IE for
hostapd). Allow this to be done with cfg80211_new_sta().

Make sure there is no pending ctrlrequest before removing the config.
Otherwise the ctrlrequest complete callback could access structures
after they have been freed. Unbind cancels pending transfers but not
ep0 requests.

rtc: Fix some bugs that allowed accumulating time drift in suspend/resume
The current code checks if abs(delta_delta.tv_sec) is greater or
equal to two before it discards the old delta value, but this can
trigger at close to -1 seconds since -1.000000001 seconds is stored
as tv_sec -2 and tv_nsec 999999999 in a normalized timespec.
rtc_resume had an early return check if the rtc value had not changed
since rtc_suspend. This effectivly stops time for the duration of the
short sleep. Check if sleep_time is positive after all the adjustments
have been applied instead since this allows the old_system adjustment
in rtc_suspend to have an effect even for short sleep cycles.

rtc: Avoid accumulating time drift in suspend/resume
Because the RTC interface is only a second granular interface,
each time we read from the RTC for suspend/resume, we introduce a
half second (on average) of error.
In order to avoid this error accumulating as the system is suspended
over and over, this patch measures the time delta between the RTC
and the system CLOCK_REALTIME.
If the delta is less then 2 seconds from the last suspend, we compensate
by using the previous time delta (keeping it close). If it is larger
then 2 seconds, we assume the clock was set or has been changed, so we
do no correction and update the delta.
Note: If NTP is running, ths could seem to "fight" with the NTP corrected
time, where as if the system time was off by 1 second, and NTP slewed the
value in, a suspend/resume cycle could undo this correction, by trying to
restore the previous offset from the RTC. However, without this patch,
since each read could cause almost a full second worth of error, its
possible to get almost 2 seconds of error just from the suspend/resume
cycle alone, so this about equal to any offset added by the compensation.
Further on systems that suspend/resume frequently, this should keep time
closer then NTP could compensate for if the errors were allowed to
accumulate.

This patch also improves some of the variable names and adds more clear
comments.

The MDM device already supports sending remote wakeup signaling over
HSIC during normal bus auto-suspend. However, this feature is
explicitly suppressed when the kernel enters system-wide suspend.
This fix allows the device to continue to send resume signaling in
order to wake the kernel out of suspend.

usbnet driver supports ethernet mode by default. Since ethernet header
is 14 bytes therefore driver makes IP header 4-byte aligned to avoid
performance hit due to IP header accesses from linux network stack becomes
unaligned.
In raw IP mode, IP header is 4-byte aligned as there is no ethernet header.
Hence there is no need for 4-byte IP header alignment. This improves the
embedded rmnet throughout on rx data path.
While testing peak throughput using iperf on downlink direction, this patch
improves the performance from 56Mbps to 99Mbps when 100Mbps was pushed.

LVS1 is now turned ON by bootloader; and APPS don't need to vote for this.
RPM was turning ON this rail irrespective of any votes, it doesn't need
to do that anymore.

A usbnet subdriver must implement the manage_power callback in order
to fully support autosuspend. This allows for the device to be
suspended when there is no active traffic, and USB remote wakeup
is used to signal a resume to the host.
Remove calls to usb_autopm_get/put from rmnet_usb_ctrl_start_rx and
rmnet_usb_ctrl_write_cmd as these functions are only (or should be)
called when the device is already active.

In case of LOGP recovery Wifi is soft reset. During soft reset,
observe that regulator_get() is not called for all regulators.
This is leading to null pointer dereference by kernel when
regulator_put() is called while turning of Wifi.

Reads performed on rmnet control devices get blocked on wait queue.
Currently, reads get unblocked only when read response is received
from mdm9x15. Hence during mdm9x15 disconnect unblock reads to return
error to QMI channels.

Gergely Kalman reported crashes in check_peer_redir().
It appears commit f39925dbde (ipv4: Cache learned redirect
information in inetpeer.) added a race, leading to possible NULL ptr
dereference.
Since we can now change dst neighbour, we should make sure a reader can
safely use a neighbour.
Add RCU protection to dst neighbour, and make sure check_peer_redir()
can be called safely by different cpus in parallel.
As neighbours are already freed after one RCU grace period, this patch
should not add typical RCU penalty (cache cold effects)

CONFIG_ATH6K_LEGACY is added to support QCA AR6003 (Non cfg80211).
AR6003 driver needs WIRELESS_EXT support. WIRELESS_EXT is going to
be deprecated so adding it to defconfig will not work. Driver needs
to mark it as dependency.

When the chip is soft reset but the PHY is not, the ethernet
link does not come up. The PHY and MAC go out of sync. Reseting
PHY starts auto-negotiation.

The WLAN driver needs to know the device serial number. This change
introduces a sysfs entry to allow userspace to configure the serial
number, and exports an API which allows WLAN and other wireless
technologies to query the serial number.

Instead of forcing the TCP/IP stack to retry the packet if
an uplink wakeup is needed, queue the packet until the
wakeup is complete. This prevents tools such as tcpdump
from showing the packet as being sent twice.

bond_alb_init_slave() is called from bond_enslave() and sets the slave's MAC
address. This is done differently for TLB and ALB modes.
bond->alb_info.rlb_enabled is used to discriminate between the two modes but
this flag may be uninitialized if the slave is being enslaved prior to calling
bond_open() -> bond_alb_initialize() on the master.
It turns out all the callers of alb_set_slave_mac_addr() pass
bond->alb_info.rlb_enabled as the hw parameter.
This patch cleans up the unnecessary parameter of alb_set_slave_mac_addr() and
makes the function decide based on the bonding mode instead, which fixes the
above problem.

Same devices can generate interrupt without properly setting bit in
INT_SOURCE_CSR register (spurious interrupt), what will cause IRQ line
will be disabled by interrupts controller driver.
We discovered that clearing INT_MASK_CSR stops such behaviour. We
previously first read that register, and then clear all know interrupt
sources bits and do not touch reserved bits. After this patch, we write
to all register content (I believe writing to reserved bits on that
register will not cause any problems, I tested that on my rt2800pci
device).
This fix very bad performance problem, practically making device
unusable (since worked without interrupts), reported in:
https://bugzilla.redhat.com/show_bug.cgi?id=658451
We previously tried to workaround that issue in commit
4ba7d9997869d25bd223dea7536fc1ce9fab3b3b "rt2800pci: handle spurious
interrupts", but it was reverted in commit
82e5fc2a34fa9ffea38f00c4066b7e600a0ca5e6
as thing, that will prevent to detect real spurious interrupts.

In 8660 wifi is using 8901 regulators. Once wifi is enabled and disabled
back, these regulator does not go in LPM mode and causing the device to
draw more current in suspend state. Hence put these regulator in LPM mode.

If bam_dmux times out waiting for a response from the modem, we assume it
has crashed. Therefore, prepare for modem restart if enabled, otherwise
report the error with BUG as we are in an unrecoverable state

Embedded rmnet driver is setting skb protocol to ethernet link layer
protocol, which causes the IP stack to drop the packets if it is set
to raw IP link layer protocol mode. Hence set the SKB protocol based
on the RMNET link layer protocol mode.

If slave device already has a receive handler registered, then the
error unwind of bonding device enslave function is broken.
The following will leave a pointer to freed memory in the slave
device list, causing a later kernel panic.

The fix is to detach the slave (which removes it from the list)
in the unwind path.

At this point if skb->len happens to be 2, the subsequant skb_pull(skb, 4)
call won't work and the skb->len won't be decreased and won't ever reach 0,
resulting in an infinite loop.
With an ASIX 88772 under heavy load, without this patch, rx_fixup() reaches
an infinite loop in less than a minute. With this patch applied,
no infinite loop even after hours of heavy load.
